Coming home from hospital
A simple checklist for the day someone you care for comes home, so the first night goes smoothly.
The day someone leaves hospital can be a relief and a worry at the same time. Plans change, discharge takes longer than expected, and suddenly you are at home without the nurses. A few preparations make the first days much easier.
The steps
Four small steps,in your own time.
Before discharge, ask the ward: what medicines are they taking now, what should we watch for, and who do we call if we are worried?
At home, make the bed and the route to the bathroom easy and clear. Put essentials within reach of where they will sit and sleep.
Have simple food in the house for a few days, and something easy for the first evening that needs no cooking.
Write the key numbers on one page by the phone: the ward, their doctor, any community nurse, and the local emergency number.
The reason
Why it helps
The first days at home after hospital are often when people feel most uncertain. Knowing the plan, having the home ready and having the right numbers to hand can make that time calmer for everyone. Asking questions before leaving is much easier than trying to reach the ward later.
A gentle variation
If you live far away from the person coming home, ask a neighbour or friend to check the house is warm and has food in it, and arrange a call at a set time each day for the first week.

WE · TRIBE
Try itwith someone.
Share the first week among family or friends: one person brings a meal, another does the shopping, another sits with the person so you can rest. Ask for specific help; people find it easier to say yes.
Try it today
Before the next discharge day, write down the three questions you will ask the ward, and keep the list in your bag.
